PRIORITY DEADLINE: APRIL 29, 2022
EXTENDED DEADLINE: JUNE 1, 2022
NOTE: WE ENCOURAGE YOU TO CUT-AND-PASTE YOUR ANSWERS TO THIS APPLICATION INTO A WORD DOC BEFORE YOU HIT THE SUBMIT BUTTON AS WE OCCASIONALLY EXPERIENCE SOME TECHNICAL DIFFICULTIES WITH THIS CONTACT FORM.  ONCE YOU HAVE RECEIVED CONFIRMATION FROM US THAT WE RECEIVED YOUR APPLICATION, THEN YOU CAN GO AHEAD AND DELETE THE WORD DOC. THANK YOU!

Because the people are the focus of the Jam, this questionnaire is longer and more in-depth than most, because we want to get to know you better. Please share as much as you feel comfortable sharing. There is no min/max length for responses and no pressure. We only ask you to share what you feel is 'enough'. We will follow up with more questions or a phone call if we need more information.

We do encourage you to take your time and share from the heart, as we use your responses not only to make an informed decision about selection, but also use them extensively to design the Jam itself.

All of your answers, including your name and basic info, will be kept in full confidentiality.* Only the organizing team will have access to your application. The more the team learns about you, the more informed their decision about selection can be and the better we can create a jam the reflects the needs of the participants.

    * Please note that if you are selected, your name and bio will be shared on the YES! website and with the other participants of your Jam prior to the event.

    Country (required)

    Phone (if applicable):

Organization or Organizational Affiliation (if applicable):

    Website of Group (if applicable):

    Birthday (Month/Day/Year) (required)

How did you hear about the Jam? Please list any and all sources by name -- We want to give them a shout-out of thanks if possible!:

How do you identify? (gender identity, race/ethnic background, sexual orientation, class, disability/ability, anything else that makes you who you are.) We ask because we value diversity, the opportunity to create safe(r) space, and want to know how to best meet participant needs:

    Who You Are

The people in the room are the focus of the Jam, and sharing our stories, experiences, ideas, etc., through different formats, is how we engage and learn together. The following series of questions is meant to be a taste of this principle. Don't worry about spelling or grammar or eloquent articulation. We just want to get a sense of who you are.

What’s does your current work (or play) look like in the world? Please be as specific as possible about what you do, why you do it, and the values behind it.

What are your creative skills, interests and passions?

Where are you in your journey as an artist-activist?

What are your burning questions in your life/work right now, especially around the intersection of arts and activism?

How do you feel this gathering may be valuable for you at this point in your life/work journey? Why do you want to come to the Jam?

What do you hope to contribute to the Jam and the community that forms at the Jam? What do you hope to receive from the Jam?

Do you have any fears, doubts or concerns about attending the Jam?

Will you be needing childcare at the Jam?

Is there anything else you'd like to tell us?

Please give us a mini-bio (a paragraph version of your story) written in the third person (for example, using "she" instead of "I"). Talk yourself up! Include contributions you've made, any awards or accomplishments, organizations you've been involved with, etc.

This is the bio that will be shared with other participants of this Jam, and will be used for the YES! Website. (You can always modify it later.) :

    Costs of Attending

The full cost of hosting this Jam is $1050 per person, of which $525 covers food, lodging and local transport and $525 covers program costs (materials, childcare, stipends for organizers and facilitators).

If you can pay the entire cost of $1050, great! And, we ask that all participants consider contributing a minimum of $525 to cover their food and accommodation. If you pay more than the at-cost $1050 amount for tuition, that extra amount is tax-deductible and directly supports low-income participants to attend.

The organizers of the Arts for Social Change Jam recognize that we come from various economic landscapes and legacies, and we are always working to deepen our commitment to making the Jam welcoming and accessible to everyone who feels called to come. We deeply believe that we are stronger together. If money is in any way a barrier for you to participate, please let us know and we will work together towards creating accessibility.

Some of the existing systems of financial support for Jams include partial scholarships, work trades, payment plans (which can be as low as $10 a month), collective fundraising and non-monetary reciprocity such as in-kind contributions. Let us know if you would like to talk about one or more of these options.

Are you or your organization in a position to pay $1050 for tuition? If not, how much are you able to contribute?

Some partial work trades may be available. Are there other non-monetary ways that you might be in reciprocity with the Jam (any particular skills, other in-kind contributions, etc.)?:

Travel expenses are the responsibility of the participant (the closest airports are San Jose, San Francisco, and Oakland CA), but we will arrange carpools from the Bay Area and airports. If you are selected, we will follow up with you to ask about diet, housing, travel plans, etc. We want to be sure to meet your needs at the Jam.

Thanks for applying to the Arts for Social Change Jam!

We’ll be in touch as soon as we can.
